چکیده

The presence of anti-lymphocytes antibodies is associated with the occurrence of acute rejection after kidney transplantation but few is known on their role after the rejection episode. We conducted a retrospective study in kidney transplant recipients who experienced a biopsy proven acute rejection episode to analyse the influence of anti-lymphocytes antibodies on clinical outcome. Anti-lymphocytes antibodies were detected before and after transplantation and characterized for isotype, class I and class II targets and donor specificity. 76 kidney recipients were included and analysed for steroid resistance of acute rejection, serum creatinine and 1-year actual graft survival. The presence of anti-lymphocytes antibodies was noticed in 80% of patients. Anti-lymphocytes antibodies were associated with more frequent steroid resistant rejection episodes, higher creatinine at discharge and throughout the first year post transplantation and with a worse graft survival, at the condition they were of the IgG isotype, donor-specific, and they recognized class II targets. We conclude that donor-specific anti-class II IgG antibodies are deleterious in the subgroup of kidney transplant recipients who develope an acute rejection.

Anti-HLA Antibodies and Kidney Allograft Outcomes in Recipients with Donor Bone Marrow Cell Infusion

Background: Anti-HLA-antibodies are known to affect the allograft survival in transplant recipient patients. Objective: The aim of this study was to evaluate the association between anti-HLA antibodies and kidney allograft outcomes, particularly in recipients with concur-rent donor bone marrow cell infusion (DBMI). Methods: Between June 2006 and May 2007, forty living unrelated donor kidney tra...

Prediction of Rejection in Renal Transplantation by Immune Parameters

Background: Monitoring of phenotypic characteristics of T-lymphocytes in peripheral blood is commonly performed to give the clinical parameters in the management of kidney transplant recipients.   Objective: To predict rejection in renal transplantation by immune parameters. Methods: 16 non-diabetic kidney transplant candidates (4 females and 12 males, age = 20-65 yr, first time transplant) wer...

Cytotoxic antibodies – valuable prognostic factor for long term kidney allograft survival

BACKGROUND Since the first attempts of kidney transplant, the inflammation mediated by T lymphocytes was considered one of the most important processes implicated in graft rejection but, multiple acute and chronic graft rejects revealed that the inflammation process is not singular and humoral mechanisms may play a role in the development of chronic vascular rejection. MATERIAL AND METHODS We...

Clinical relevance of anti-HLA antibodies detected by flow-cytometry bead-based assays--single-center experience.

The purpose of this study was to define the incidence, dynamics, and profiles of anti-human leukocyte antigen antibodies (HLA-Abs) produced after kidney transplantation and their impact on graft outcome. A total of 72 first cadaver donor kidney recipients were prospectively monitored for the development of HLA-Abs using bead-based flow-cytometry assays (One Lambda FlowPRA tests). Sixteen recipi...

Humoral immunity in hand transplantation: anti-HLA and non-HLA response.

UNLABELLED Antibodies against donor's HLA antigens and B cell activity are recognized modulators of immune response to allograft. The role of both anti-HLA and non-HLA antibodies is understood in solid organ transplantation, but has not been addressed in composite tissue allografts. AIM We decided to evaluate the presence and role of anti-HLA and non-HLA antibodies after hand transplantation....
